Times allow for collecting and dropping off, giving your dog a one hour walk time.

Does your dog have a collar with a name tag?

Is your dog at all aggressive (towards other dogs/people etc)

In winter months or when raining, your dog will be towel dried on return. Please leave towels by your entrance for this. Tails & Trails will endeavour to make your dog as clean/dry as possible but cannot guarantee cleanliness, so please ensure your dog can be left in an area away from soft furnishings if you do not wish for your dog to make a mess in your absence.

This is especially important for those with active dogs (dirt collectors) ie spaniels/labs/retrievers etc. Tails & Trails can take no responsibility for damage made by wet/mucky dogs.

If you have a dry robe/equa fleece etc, you wish your dog to wear on return, please leave with towels.

Please ensure your vet is aware that someone other than yourselves may bring your dog(s) to them in an emergency.

During my absence Tails & Trails has permission to transport my dog(s) to the surgery for emergency treatment. I authorise treatment and I as the dog(s) owner will be responsible for the cost of treatment, not Tails & Trails.

I hereby give Tails & Trails permission to transport my dog(s) to the above-mentioned surgery and to make decisions on treatment in my absence (if I cannot be contacted) I understand Tails & Trails assume no responsibility for the loss of the dog(s) and is released from all liability related to transportation, treatment and expense. This will be my responsibility as the dog(s) owner.

Dogs which are off lead only

I agree for my dog to be let off lead. I do not hold Tails & Trails responsible if my dog(s) is lost or stolen, or if they endanger themselves when off lead or should they run away.

In the event of extreme heat/heavy snow

If weather makes walking dangerous, ie too hot, we will provide a drop-in service where each dog will have a visit for a wee/poo and some company for fusses. This will be charged at £8.00 per visit.

If weather is extreme and makes driving dangerous ie heavy snow, walks will be cancelled and no charge made. The likelihood is, if we can’t get to you, you probably can’t get out either.

Group dog walks will be charged at £15.00 per dog per hour.

Prompt payment following receipt of invoice at the end of the month will be appreciated.

The information I have given in this application is true, correct and complete to the best of my knowledge. I agree to the terms set above. I understand this form acts as permission to hold keys to my property which I have given willingly. I hereby indemnify Tails & Trails and their staff against any liability of any kind whatsoever arising from damage, loss, disappearance, injury, death to a pet either inside or outside the house whilst in their care (also includes to property) I also give Tails & Trails permission to transport my dog(s)

I agree to make payment for the services provided following receipt of invoice at the end of each calendar month.

Tails & Trails is fully insured and all walkers hold DBS certificates.
